Racional: A ocorrência do divertículo faringoesofágico, ou de Zenker, é pouco frequente no cenário nacional, sendo que a técnica da diverticulectomia com cricomiotomia em divertículos de média e grandes dimensões ainda é a mais indicada. Devido à ressecção do divertículo necessitar de sutura da faringe ocorre possibilidade de deiscência, o que retarda o retorno da deglutição. Daí a ideia de realizar este procedimento cirúrgico, comparando a sutura manual com a mecânica, para avaliar o real benefício da técnica mecânica. Objetivo: Avaliar os resultados da diverticulectomia faringoesofágica com cricomiotomia utilizando à sutura manual e mecânica em relação às complicações locais e sistêmicas. Métodos: Foram estudados 57 pacientes com divertículos faringoesofágicos diagnosticados através da endoscopia digestiva alta e faringoesofagograma. A técnica cirúrgica empreendida foi a diverticulectomia com miotomia do músculo cricofaríngeo, sendo a sutura mecânica realizada em 24 pacientes (42,2%, grupo A) com o aparelho linear e em 33 (57,8%, grupo B) a manual para o fechamento da faringe. Resultados: Na avaliação do pós-operatório precoce, um paciente do grupo A (4,1%) apresentou fístula consequente à deiscência da sutura da faringe e três do grupo B (15,1%) ambos com boa evolução com tratamento conservador. Neste mesmo grupo, três pacientes (9,0%) apresentaram estenose da sutura da faringe, com boa evolução com dilatações endoscópicas sendo que nenhum do grupo A apresentou esta complicação. A infecção pulmonar esteve presente em cinco pacientes, dois (8,3%) do grupo A e três (9,0%) do grupo B, com boa evolução com tratamento específico. Na avaliação tardia, realizada em 43 pacientes, 17(94,4%) do grupo A e 22 (88,0%) do grupo B, os pacientes referiram estarem satisfeitos com o procedimento cirúrgico, pois conseguiram resgatar a deglutição normal. Conclusões: A diverticulectomia com a miotomia do cricofaríngeo demonstrou ser procedimento cirúrgico adequado por restaurar deglutição adequada e o fechamento da faringe com sutura mecânica mostrou-se técnica mais eficaz em relação à sutura manual, com menor índice de complicações pós-operatórias locais

Background: The occurrence of the pharyngoesophageal, or Zenker diverticulum is not frequent in the national scenario, and the technique of the diverticulectomy with cricomyotomy in medium and great dimension diverticula is still the most indicated. Because the resection of the diverticulum requires the suture of the pharynx, dehiscence can occur, thereafter delaying swallowing. Hence, the idea is to accomplish this surgical procedure, comparing the manual and mechanical suture, in order to evaluate the real benefit of the mechanical technique. Aim: To evaluate the results of the pharyngoesophageal diverticulectomy with cricomyotomy using manual and mechanical suture with regard to local and systemic complications. Method: Fifty-seven patients with pharyngoesophageal diverticula diagnosed through high digestive endoscopy and pharyngeal esophagogram were studied. The applied surgical technique was diverticulectomy with myotomy of the cricopharyngeal muscle, done in 24 patients (42.2%) the mechanical suture (group A) with the mechanical linear suture device and in 33 (57.8%) a manual closure of the pharynx (group B). Results: In the postoperative period, one patient of group A (4.1%) presented fistula caused by dehiscence of the pharyngeal suture, and three of group B (15.1%) presented the same complication, with a good outcome using a conservative treatment. In the same group, three patients (9.0%) presented stenosis of the suture of the pharynx, with good outcome and with endoscopic dilatations, and no patient from group A presented such complication. Lung infection was present in five patients, being two (8.3%) of group A and three (9.0%) on B, having good outcomes after specific treatment. In the late review, done with 43 patients (94.4%) of group A and 22 (88.0%) on B, the patients declared to be pleased with the surgical procedure, because they were able to regain normal swallowing. Conclusion: The diverticulectomy with myotomy and pharyngeal closure using mechanical suture was proven appropriate, for having restored regular swallowing in most of the patients, and the mechanical closure of the pharynx proved to be more effective in comparison to the manual one, because it provided a lower index of local post-surgical complications.

RACIONAL: O divertículo faringoesofágico apresenta-se como distúrbio pouco frequente e é provável que este fato seja responsável pela dificuldade encontrada na escolha do método terapêutico mais eficaz. O tratamento cirúrgico ainda é a principal opção terapêutica. No entanto, há controvérsias quanto à técnica cirúrgica que garante melhores resultados. OBJETIVO - Avaliar a eficácia da diverticulectomia com miotomia do músculo cricofaríngeo pela sutura mecânica linear no tratamento cirúrgico do divertículo faringoesofágico em uma série de pacientes no tocante às suas complicações locais e sistêmicas. MÉTODO: Foram atendidos 19 pacientes com divertículo de Zenker. Todos apresentavam as condições clínicas necessárias e preenchiam os critérios de elegibilidade para realização do procedimento cirúrgico. Após a diverticulectomia com sutura mecânica todos foram submetidos a avaliação pós-operatória no sentido de identificar a manifestação de qualquer tipo de complicação. Os pacientes foram estudados retrospectivamente através da análise de prontuários e descrição da internação hospitalar realizada no momento da operação e da evolução ambulatorial. RESULTADOS: A totalidade apresentou resultados satisfatórios, sem evidência de fístula ao nível da sutura da faringe no período pós-operatório precoce. Somente dois indivíduos perderam seguimento na avaliação tardia e um desenvolveu estenose faríngea revertida com dilatação endoscópica. Todos referiram estar satisfeitos com o procedimento empregado e relataram melhora na qualidade de vida ao final do seguimento, proporcionada pelo retorno da deglutição. CONCLUSÃO: A diverticulectomia com a miotomia do músculo cricofaríngeo e posterior fechamento da faringe com sutura mecânica linear mostrou-se técnica eficaz, com poucas complicações pós-operatórias e melhora significativa na qualidade de vida.

BACKGROUND: Pharyngoesophageal diverticulum presents itself as an uncommon disorder. Thus, choosing the most effective treatment method may be challenging. Surgical treatment remains as the main option. However, establishing the best surgical technique is still controversial. AIM: To evaluate the results of diverticulecomy with chricopharyngeal miotomy and linear stapler closure of the pharynx in a patient sample with Zenker`s diverticulum regarding local and systemic complications. METHODS: Nineteen patients with pharyngoesophageal diverticulum were admitted. All of them presented the clinical conditions required to indicate the surgical procedure. Patients were evaluated with regard to any post-operative complications. This study was conducted retrospectively with patients' data analysis. RESULTS - Patients showed satisfactory results, with no evidence of fistula at the level of the pharyngeal suture. Two out of 19 lost post-operative follow-up and one of them had pharyngeal stenosis reverted with endoscopic dilation. The entire sample reported being satisfied with the procedure emphasizing improvement of the quality of life mainly due to the return of physiological deglutition. CONCLUSION: The diverticulectomy with myotomy and posterior pharyngeal closure with linear surgical stapler proved to be an effective technique, offering a lower risk of post-operative complications and improving the overall quality of life.

O tratamento cirúrgico de escolha no carcinoma diferenciado da tireóide sempre foi controverso. OBJETIVO: Analisar o acometimento tumoral do lobo contralateral da tireóide no carcinoma diferenciado, correlacionando risco e benefício com as complicações decorrentes da segunda intervenção. CASUÍSTICA E MÉTODO: Estudo retrospectivo, de 1998 a 2006, com 27 pacientes submetidos à tireoidectomia menos que total, sendo 21 lobectomias, cinco tireoidectomias subtotais e uma istmectomia. Foram analisados: gênero, idade, tipo de cirurgia, complicações, histopatológico do espécime cirúrgico e invasão do lobo contralateral. As idades variaram de 17 a 89 anos; o tipo histopatológico mais freqüente foi o carcinoma papilífero clássico (18 casos), seguido do carcinoma folicular (seis casos), do carcinoma papilífero variante folicular (dois casos) e do carcinoma de células Hürthle (um caso). Vinte e um pacientes foram submetidos à totalização da tireoidectomia, 15 a 30 dias depois. RESULTADOS: A análise do lobo contralateral foi negativa para carcinoma em 16 (76,5%) e positiva nos cinco restantes (23,8%). As complicações observadas foram: disfonia temporária (três casos) e hipoparatireoidismo (dois casos, sendo um permanente). CONCLUSÃO: A totalização da tireoidectomia é um procedimento importante no tratamento do carcinoma bem diferenciado da tireóide pelo elevado acometimento contralateral (23,8%). A incidência de complicações é pequena.

The treatment of choice for the well differentiated thyroid carcinoma has always been controversial. AIM: to analyze tumor invasion of the thyroid gland's contralateral lobe in cases of differentiated carcinoma, correlating risk/benefit with the complications of a second surgical approach. MATERIALS AND METHODS: Retrospective study, from 1998 to 2006, of 27 patients undergoing less than total thyroidectomy: lobectomy (21), subtotal thyroidectomy (5) or isthmusectomy (1). Gender, age, type of surgery, complications, histopathological analysis and invasion of the contralateral lobe were analyzed. Patients' ages varied from 17 to 89; the most frequent histopathological pattern was the classical papillary carcinoma (18 cases), followed by follicular carcinoma (6); the follicular variant of the papillary carcinoma (2) and the Hürthle cell carcinoma (1). Twenty-one patients underwent full thyroidectomies, from 15 to 30 days after the first intervention. RESULTS: the contralateral lobe analysis was negative for carcinoma in 16 (76.5%) and positive in the other 5 (23.8%) patients. The complications observed were temporary dysphonia (3 cases) and hypoparathyroidism (2 cases, one permanent). CONCLUSIONS: total thyroidectomy is important in the treatment of differentiated thyroid carcinomas, because there is a high contralateral spread rate (23.8%). It is a procedure without mortality, which bears few complications.

TEMA: treinamento fonoaudiológico para pacientes laringectomizados totais com prótese traqueoesofágica. OBJETIVO: comparar a qualidade da comunicação oral de pacientes laringectomizados, com prótese traqueoesofágica antes e depois de treinamento fonoaudiológico. MÉTODO: foram analisadas amostras de voz e fala de 17 pacientes laringectomizados, antes e depois de treinamento de voz e fala. As amostras pré e pós-treino foram misturadas ao acaso e apresentadas a fonoaudiólogos para avaliação. RESULTADOS: observada melhora significativa (Teste da Binomial) no conjunto dos aspectos avaliados. CONCLUSÃO: pode-se considerar que o treino proposto melhorou os padrões de fala inclusive a melodia e do canto.

BACKGROUND: speech and voice training for laryngectomized patients with tracheoesophageal prosthesis. AIM: to compare the quality of oral communication of a group of laryngectomized patients with tracheoesophageal prostheses, pre and post speech and voice training. METHOD: voice and speech samples of 17 laryngectomized patients were analyzed before and after receiving speech and voice training. The voice and speech samples were mixed and presented to speech-language pathologists for assessment. RESULTS: statistically significant (Binomial Test) improvement was observed. CONCLUSION: it can be concluded that the proposed training improved the speech patterns, including melody and singing.

OBJETIVO: Avaliar retrospectivamente os resultados de pacientes submetidos a laringectomias parciais por carcinoma epidermóide de laringe no serviço do HMCP. MÉTODOS: Dos doentes estudados, 28 eram do sexo masculino (93,5%) e dois do feminino (6,5%) com idade variável de 42 a 66 anos, com média de 58 anos. Em relação ao procedimento cirúrgico, este foi variável, dependendo do local da extensão tumoral e do estádio. A técnica mais utilizada foi a laringectomia frontolateral em 12 pacientes (40%), seguido pela laringectomia vertical em oito (26,6%), laringectomia supraglótica em quatro (13,3%), laringectomia supracricóidea em quatro (13,3%), laringectomia near-total em um (3,3%) e cordectomia em um (3,3%). RESULTADOS: Na avaliação pós-operatória precoce, três pacientes (10%) apresentaram complicações evidenciadas por fístula faringocutâneas em dois, com boa evolução após tratamento conservador, e pneumotórax no paciente restante com boa evolução após drenagem do tórax. No que se refere à avaliação da deglutição, cinco pacientes (16,6%) apresentaram dificuldade de ingestão oral devido a aspirações recorrentes com tempo variável de 45 a 180 dias (média - 65 dias), o que obrigou a um paciente ser submetido à totalização da laringectomia devido a pneumonias recorrentes. Na avaliação tardia, de seis a 125 meses e média de 29 meses, quatro pacientes (13,3%) apresentaram recidiva locorregional, sendo dois submetidos à laringectomia vertical, um à frontolateral e o restante à supracricóidea. Em todos foram realizadas totalização e/ou radioterapia, e dois evoluíram a óbito pela doença. Vinte e quatro pacientes (84%) apresentaram preservação da voz. CONCLUSÃO: Assim, os autores concluem que a cirurgia conservadora da laringe proporciona bom controle da doença e com sobrevida aceitável, já que 90% dos pacientes estudados estão vivos e sem doença até o final da avaliação. Além disso, apresentam melhor qualidade de vida já que a maioria dos pacientes demonstrou voz e ingestão oral bem satisfatórias.

BACKGROUND: This is of a retrospective study of the results obtained from partial laryngectomy for epidermoid carcinoma. METHODS: Twenty-eight males (93.5%) and 2 females (6.5%), between 42 years and 66 years old (mean age, 58 years). The surgical procedure varied in accordance to the site, extension and stage of the tumor. The technique most used was the frontolateral laryngectomy, performed on 12 patients (40%), followed by the vertical laryngectomy on 8 patients (26.6%), the supraglottic laryngectomy on 4 patients (13.3%), supracricoid laryngectomy on 4 patients (13.3%), near total laryngectomy on 1 patient (3.3%) and chordectomy on 1 patient (3.3%). RESULTS: An early postoperative assessment showed that 3 patients (10%) had complications. Two of them had pharyngocutaneous fistulas that responded well to conservative treatment and the remaining patient had pneumothorax, which improved after thoracic drainage. Deglutition assessment revealed that 5 patients (16.6%) presented difficulty in oral ingestion as a result of recurrent aspirations that occurred at periods that varied between 15 days to 180 days (average 180 days) and caused recurrent pneumonia. The only remaining option was total laryngectomy. The next assessment was performed after a period that varied between 6 months to 125 months (mean of 29 months) and revealed that 4 patients (13.3%) had recurrence in the same region. Two of these patients underwent vertical laryngectomy, 1 patient had a frontolateral laryngectomy and the remaining patient underwent supracricoid laryngectomy. All patients underwent total laryngectomy and/or radiotherapy, but the disease caused the death of 2 patients. Twenty four patients (84%) demonstrated that their voice was preserved and at the end of the assessment, 90% of the patients were alive and without any signs of the disease. These patients had a far better quality of life as they presented fairly satisfactory voice and oral ingestion. CONCLUSION: The conservative surgery of the larynx provided good disease control and increased the patient's life span.

OBJETIVO: Analisar a invasão tumoral do lobo contralateral da glândula tireóide no carcinoma diferenciado, correlacionando o risco/benefício com as complicações decorrentes de uma segunda intervenção. MÉTODO: De outubro/93 a dezembro/96 foram operados 20 pacientes com carcinomas diferenciados da glândula tireóide. Os parâmetros analisados foram sexo, idade, tipo de operação, tipo de complicações, histopatológico da peça cirúrgica e invasão do lobo contralateral. Eram dois pacientes do sexo masculino (10%) e 18 do feminino (90%); as idades variaram de 17 a 89 anos; o tipo histológico mais freqüente foi o carcinoma papilífero (13 casos), seguido do folicular (seis casos) e carcinoma de células de Hürthle (um caso). Como primeiro procedimento cirúrgico houve 11 lobectomias + istmectomias, quatro lobectomias subtotais e uma istmectomia. Cinco pacientes não realizaram a totalização (um por fibrose, três por perda de seguimento e um por ser microcarcinoma). RESULTADOS: Na análise do lobo contralateral realizada em 15 pacientes, 11 resultaram negativas e outras quatro positivas (26,6%). As complicações apresentadas foram rouquidão (dois casos revertidos com tratamento fonoterápico), hipoparatireoidismo (dois casos, um transitório e um permanente). CONCLUSÃO: A totalização da tireoidectomia é um procedimento importante no tratamento do tumor maligno da tireóide pela alta porcentagem de metástase contralateral (26,6%). Além disso, é um procedimento com mortalidade nula e pequena incidência de complicações.

BACKGROUND: Our goal is to analyze tumor invasion of the contralateral thyroid lobe, evaluating the relationship between risk/benefit with complications of a second surgery. METHOD: From October, 1993 to December, 1996 twenty patients with thyroid tumors were operated. The analysed variables were sex, age, type of surgery, complications, histophatologic examination ot the surgical especimen and contralateral lobe invasion. There were 2 men and 18 women; the age ranged from 17 to 89 years old; the most common histologic type was papillary carcinoma (13 cases) followed by follicular carcinoma (6 cases) and Hürtle cell carcinoma (1 case). The initial surgical procedures were 11 lobectomies plus isthmectomies; 4 lobectomies; 3 subtotal thyroidectomies; 1 total thyroidectomy and 1 isthmectomy. Five patients were not submitted to total thyroidectomy (1 by fibrosis, 3 by loose of follow-up and 1 case of microcarcinoma). RESULTS: Analysis of the contralateral lobe (performed in 15 patients) showed that 11 were negative and 4 positive (26,6%). Complications observed were dysphonia (2 cases treated with fonotherapy), and hypoparathyroidism (1 transient and 1 permanent). CONCLUSIONS: The totalization of thyroidectomy is an important procedure for the treatment of malignant tumors of thyroid due by the high rate of contralateral metastasis (26,6%). Moreover, it's a procedure without mortality with lower complications.

A maioria dos retalhos utilizados na reconstrução dos grandes defeitos pós-tratamento cirúrgico isolado ou associado à radioterapia no câncer da cabeça e pescoço levou os autores a estudar a viabilidade do RMF em 14 pacientes, atendidos no Serviço de Cabeça e Pescoço do Hospital Celso Pierro, PUCCAMP, Campinas. Todos eram portadores de lesões epidermóides avançadas no estádio IV, localizadas na boca (dez), hipofaringe (dois), laringe (um) e pescoço (um). Através do teste exato de Fisher (p< 0,05), aferiu-se a existência ou não da relação da viabilidade do RMF com a idade, estado nutricional, tratamento prévio. Nestes 14 pacientes, foram empregados 16 retalhos, 13 deles viáveis (81,2%) e três (18,8%) com necrose total, não havendo associação entre a viabilidade e a necrose e a idade (p=1,0). Quanto à relação com o tratamento prévio com quimioterapia (três casos) e radioterapia (dois casos), estes não foram significantes (p=0,547).

The great incidence of advanced squamous cell carcinoma of the mouth, pharynx, larynx and neck, justifies the extensive resections that are carried out by the authors at the Head and Neck Service of Hospital Celso Pierro PUCCAMP - São Paulo. Major pectoralis myofascial flap were used to manage 14 patients with advanced head and neck squamous cell carcinoma of the mouth (ten cases), pharynx (two cases); larynx (one case) and neck (one case). There was no complications in 81.2% and in 18.8% total necrosis of the flap was observed. We conclude that viability, age and necrosis or fibrosis were not associated with the success of this procedure (p=1.0), and previous treatment (chemoradiotheraphy and or radiotherapy alone) had no significant relation with flap viability (p=0.547).

OBJETIVO: avaliar a importância da experiência do cirurgião nos índices de complicação pós-operatória e nas taxas de recidiva e de sobrevida. MATERIAL E MÉTODOS: foram comparados os resultados obtidos pelos 8 cirurgiões especialistas integrantes do quadro permanente do Serviço e que possuíam graus variados de experiência considerando o tempo de exercício da especialidade. RESULTADOS: os resultados observados não apresentaram diferença significante entre os vários cirurgiões e esta uniformidade é explicada pelo fato de que todo o planejamento terapêutico foi elaborado por consenso de todo o grupo e isto pode ter minimizado uma menor experiência do cirugião responsável pela condução do ato operatório. CONCLUSÕES: os autores enfatizam a importância da avaliação pré-operatória como uma condição para que bons resultados sejam esperados e propõem que é na direção do completo domínio da propedêutica da região que devem ser dirigidos os programas de formação de novos especialistas.

This is a prospective study supported by 170 cases of epidermoid carcinoma of the larynx or hypopharynx, treated during the period from January of 1981 to January of 1988, at the Head and Neck Surgery Service of the Heliópolis Hospital Complex, São Paulo. The objective of this project was to evaluate the importance of surgeon experience with regard to the rates of post-operative complications and the rates of relapse and survival. The results of the 8 surgical specialists who integrate the permanent staff at the institute and who different varying degrees of experience with regard to time spent exercising their specialties were compared. The results obtained did not show a significant difference among the various surgeons, and this uniformity is explained by the fact that all the therapeutic planning was elaborated through consensus of the whole group, and this could have minimized the effect of experience of a surgeon responsible for the operation. The authors emphasize the importance of pre-operative evaluation for good results and propose that it is in the direction of complete mastery of preliminary work in the area that programs for the formation of new specialists should be directed.
